I replaced the conductor plate as instructed by a trans shop. I had ratio codes. The problem went away for 2 weeks. Now I am back in limp mode. No codes this time. Fluid level is correct. New filter and oil. If I disconnect the battery for awhile I can drive the car a few blocks then it goes right back into limp mode


Also - When limp mode occurs the car makes it about a 1/4 mile and stops moving all together while in gear. No reverse either. 

Did you also change the 13 pin connector?

What codes are coming up when scanned with a scanner that can retrive transmission codes?

Yes I changed it. It came with the kit. I have the OB2 sensor which read "improper gear ratio" originally. That cleared itself after installing the new conductor plate. I drove it quite a bit after changing it out. It lasted 2 weeks. Now it goes into 'limp mode' after just a few blocks. No codes this time. 

I ordered the MB II Unit to read and erase trans codes. I'm praying the torque converter is OK. 

Clear the codes with the scanner but also check the transmission fluid level is correct. It could be low throwing the car into limp mode.

Just noticed that you wrote 35k miles in the title. These transmissions are bulletproof. Never seem one fail at such low miles. As stated earlier, check the fluid level.
